At PHP, we work hard to help you get the coverage, care, and personal support you deserve, without all the hassle. That means ensuring you have access to the care prescribed by your Physician, hospital care team, and healthcare providers to achieve your healthcare goals. The PHP Case Management program steps in to help following an inpatient hospital stay, at the request of your Physician, or for those members with complicated health situations.

What can you expect?

A PHP Case Manager will call you to discuss your healthcare needs. Together with you, your family, and your doctors, we will formulate a plan to meet your healthcare goals and we will follow up to ensure they are met. We advocate on your behalf to find resources and connect you with the services you need, including:

  • Medications
  • Home Health Care
  • Rehabilitation Services
  • Finding a Primary Care Physician
  • Referrals when seeking care from an Out-of-Network Specialist
  • Education on your condition
  • Community resources
  • Transplant support
  • Behavioral Health
  • Substance Use Disorders (SUD)
